Name of the Procedure: Renal scan
(Function) with Lasix Renogram.
-
Indications: The test evaluates blood
flow to the kidneys. Onset hypertation; infection;
obstruction;
Information about renal size, location and anatomy;
renal artery stenosis; Tumors.

Contraindications: none
-
Patient Preparation: Patients should
be well hydrated before the test. If an oral ACEI is
used, patients should drink only water and should not eat a
solid meal within 4 hrs of the study. Dehydration and
over-hydration should be avoided. Patient’s history should
be obtained and include any history of cardiovascular
disease, medications, and the efficacy of blood pressure
control. A sitting and standing blood pressure and heart
rate should be measured before the exam, and at the
conclusion of the test.

Radiopharmaceuticals: Tc-99m MAG3;
Administered activity is 5-10 mCi; Rout e of administration
is intravenous injection; bolus injection in antecubital
vein. Injection to imaging time is immediate.

Adjunct Pharmaceutical: none
-
Acquisition Parameters: The
study is acquired with the gamma camera facing the lower
back of supine patient. (View: posterior. For transplant,
anterior view is taken). Low energy, all-purpose, parallel
hole collimator is used or pinhole if needed. Photo-peak
selection should be 140 keV with 20% window. Kidney and
bladder should be visualized even though it is better to
include heart in the field of view. Matrix resolution is
preferably 128 x 128, but 64 x 64 can also be used.

Renal Scan (Function): Imaging time
takes 30 min. 15 Dynamic images; 2 min each are
obtained.

Furosomide (0.3 mg/kg) is given
intravenously about 15 min in to the study if excretion
delay is seen. After giving Fursomide, continue with
dynamic images for about 10 more frames. Take the
pressure of the patient before injecting Lasix.

Computer Processing: Choose
patient’s name; then click renal processing; Click ROI; then
click ROI tool; draw region of interest around the left
kidney; then draw the ROI around the right kidney; click ok;
then click flexible display tool; then click result
composite; click hardcopy; click result/display, and print
the picture.

Normal Result: Imaging reveals
findings compatible with normal blood flow to the kidneys.
Normal blood flow through the vessels is desired. Normal
size, shape, position, and function of both kidneys are
found. Normal kidney function should be more than 35% each
kidney.

Abnormal Result: Abnormal results
indicate reduced kidney function. Abnormal size, shape, and
position of the kidney. Abnormal kidney function is less
than 35% each kidney.
